What is HOSPICE?
Hospice is specialized care for patients facing a life-limiting illness.
Our multidisciplinary team develops and carries out an individualized plan of care to meet each patient’s unique medical, emotional, and spiritual need. We provide a home-based clinical service that addresses quality of life with an emphasis on pain management and symptom control. Our multidisciplinary team of physicians, nurses, social workers, chaplains and home health aides work together with the patient and families to ensure their wishes are met with respect and dignity.
When should we start thinking about hospice care?
Deciding on hospice can be a difficult decision. In retrospect, many families wished they had started hospice care for their loved one earlier. Eligible hospice patients who have decided to discontinue aggressive, curative treatments may benefit from the gift of hospice.

Where do I go to get hospice?
Hospice care is a home-based service. Whether the patient lives at a home, skilled nursing facility, assisted living facility, retirement community or board and care, our clinical staff, durable medical equipment, supplies and medication will be delivered to wherever you call home.
Who pays for hospice?
Hospice is covered 100% by Medicare and MediCal for all patients meeting the hospice criteria. Most commercial insurance plans also have hospice coverage. A hospice evaluation is free and a clinical assessment will help you determine if hospice is the right choice for you or your loved one. At any time, you may discontinue hospice and go back to your original insurance plan.
How do I start the process?
A phone call by the patient, family member or physician at (562) 924-1555 can initiate a free hospice evaluation. Prudential Hospice Care is open twenty-four hours a day, seven days a week. Our RN case manager will be your healthcare advocate to help navigate your healthcare options. If the decision for hospice is the best course of action for the patient and family, the RN case manager will gather medical information and contact the patient’s physician for a hospice order and consent to admit to hospice.
How long before I am admitted to hospice?
Once the initial assessment concludes and the decision for hospice care is made, the response time is within a few hours. Prudential Hospice Care will work with the family’s schedule in order to make it as collaborative and convenient as possible.
What should I expect once admitted to hospice?
The RN case manager along with a physician will collaborate to design a plan of care tailored to the needs of the patient. Your physician is welcome to continue care during hospice or our dedicated hospice physician. A visit from a social worker or chaplain is available along with baths from our certified home health aides. All durable medical equipment, supplies and medications will be delivered to the location of the patient.
